Initial Client Assessment


Before You Arrive

Intake Form

One of the most important things you can do is have your MD or chiropractor write a prescription for you to have Active Release Therapy (ART) and massage therapy. This way, you can avoid paying the 7% Ohio sales tax on the services rendered at Peak Performance Sports Therapy.

Please bring this form with you on your initial visit, along with copies of any MRIs, CT scans or x-rays you have had completed. (Andy -- do you still request people fill out a Pain Referral Chart before appts? If so, can you provide that verbiage/link?)

Scheduling Your Appointments

Generally I schedule patients 3-4 weeks out, so during our first contact, I will ask you to suggest two or three appointment times that fit your schedule. I will do my best to fit you in, and depending on how long you have endured your particular pain or injury, we will schedule you for three to five sessions in advance. Although you will notice a difference after your first session, it will likely take multiple sessions to obtain the results you seek. The longer you have suffered from the injury, generally the more sessions it will require to treat the scar tissue and relieve your pain.

To most effectively and efficiently treat your pain, I recommend allowing no more than a week or two between your initial visit and your next session. This will give you the most lasting benefit and will help you feel stronger faster. I will do my part to make sure this is possible, as your body will not recover as well if there is a long lag-time between your first and second sessions.

Comfortable Clothing

You will be participating in a dynamic, movement-based therapy, so comfortable clothing is essential. Please wear loose shorts and a sports top to obtain the most effective therapy. Compression shorts are too thick for me to achieve the proper depth required to effectively release the tissue so save those for when you have recovered. Please refrain from using lotions or oils on your skin prior to your appointment to help ensure good contact.


During First Session

Communication

Relieving your pain is a true collaborative effort, and like all good collaborations, communication is essential. I will help you remember that although we are working to make your pain disappear, it is important you not withstand too much discomfort, especially during your first session.

I will continually check in with you to be sure your discomfort level hovers between a level 3 to level 6 (Mild to Moderate on a scale of 1-10, 10 being severe pain). Considering the amount of toxins, waste material, and unhealthy tissue that will be released during your first session, and the increased circulation of healthy, oxygen-rich blood flow; you will be sore for up to two or three days following your initial session.

The good news is, you will experience significantly less discomfort following your subsequent sessions, despite increasing the depth of the work. There is no need to “tough it out” too much during your initial visit.


After First Session

Rehydrate

Did you know up to 20% of an individual’s pain pattern can be caused by dehydration? Replenishing your body with water is not only critical to your overall health, but also in moving toward living pain-free.

To quantify how much water you should drink daily, strive to drink half your body weight in ounces. For example, a 150 pound person should drink 75 ounces daily. In addition, you will need to offset the dehydrating effects of caffeine or alcohol if you drink those: double the amount of ounces of caffeine or alcohol, and add that amount of water to your daily dose.

Pain

You can expect to experience some pain for up to two or three days following your initial session. Please keep a daily journal or log observing your pain patterns during this time, either on your phone or on paper, to share with me at your subsequent session. Consider your level of pain, what time it started and whether it increased throughout the day, the area where your pain began and whether it traveled to other areas.

These details will help me tailor each subsequent session to your exact needs and requirements. Your pain level and patterns will likely shift as we work together to reduce and eliminate your pain.
